Health care coverage focused on you
Banner|Aetna is transforming health care in Arizona, creating a revolutionary system to fit your needs through:
- Easier access to doctors and care
- Simpler ways to communicate directly with your doctors
- Accessible tools to track your health care progress
- A proven dedicated customer service team
We put your doctor in the driver seat of your personal health strategy, which should increase positive health outcomes and lower costs.
With Banner Health Network at the helm of your network options, Banner|Aetna can focus more on your needs while still offering you affordable health care. You’ll have access to local doctors, including generalists, allergists, cardiologists, rheumatologists, urologists...basically any specialist you’re looking for!
It starts with the primary care physician (PCP) that you choose to lead your care team. He or she will get to know you and your medical history, and see you for regular preventive visits. If you get sick, your primary care physician will not only treat you but will also help guide your health decisions and direct your care across other network specialists and facilities you may need to use.
Your Banner|Aetna Performance network includes:
- 3,250+ primary care doctors
- 23,780+ specialists
- 44 hospitals
- 178 urgent care centers
- 15 Banner health centers
- 36 walk-in clinics
You should also keep in mind that there’s no coverage outside this network except in an emergency.

When life’s challenges get overwhelming, you don’t have to go it alone. It’s OK to ask for help. In fact, it’s the brave and smart thing to do. What’s more, you have options for getting the support you need, so you can choose what works best for you.

First, choose a primary care physician who belongs to the Banner|Aetna network.
Your PCP provides basic and routine care, including preventive exams and screenings. When you need specialty care, your PCP can refer you to network specialists. He or she will give you a referral. This referral form is important.
You have your own care team, led by your PCP.
Your team includes doctors, nurses, health coaches and other clinicians working toward your good health. One of their goals is to help you stay healthy and improve your health, not just treat you when you’re sick.
Your team communicates and coordinates.
They can see treatment provided by other doctors, access your electronic health record to see services and medications you’ve received, check lab results and view the “big picture” of your care – and your health.
You get better care for chronic conditions.
Your team creates a personalized care plan and helps you stay on track with medications, treatments and actions you can take that are designed to help you live healthier. They stay in touch to monitor your progress and adjust your plan as needed.
The plan pays benefits for in-network care only.
Remember, you'll save money and get more coordinated care when you stay in the Banner|Aetna plan network. In true emergencies, you'll have coverage for out-of-network care. But, if you go out-of-network for routine or non-emergency care, just know that you'll be responsible for the entire cost.
We recommend you get a referral from your PCP to see a specialist.
A referral is good for up to 60 days. Except for emergencies, it’s important to get authorization before receiving treatment at a hospital or before outpatient surgery. Your PCP or specialist can help with that.
You pay a flat fee (copay) for most covered outpatient services.
For some services, you may need to meet a deductible first, then pay the appropriate copay.
